In the last decade, electronic products containing nicotinecigarettes have become the most popular tobacco and nicotine adolescent delivery modality and were introduced to the United States tobacco market in 2007 as a smoking cessation tool. E-cigarettes have gained huge popularity among teenagers, and vaping has become widespread among middle and high school students.

Back when e-cigarettes were launched in 2003, one of the first studies found that vaping is 95% healthier than smoking. Although limited but recent studies have questioned this fact and it has been shown that the use of electronic cigarettes is associated with respiratory symptoms (cough or phlegm), regardless of the status of cigarette smoking, although the risk could be lower than that of cigarette smoking.

However, an observational study published last year compared lung function among vapers, cigarette smokers, or those who didn’t smoke any of the above. It revealed that vaping has similar detrimental effects on lung function as smoking, and furthermore, increasing vaping use among youth could stimulate cigarette use, creating a generation dependent on smoking.

In an interview with HT Lifestyle, Dr. Sanket Jain, Chest Physician – Pulmonologist at Masina Hospital in Mumbai, revealed: “E-liquid aerosols are associated with direct damage to the respiratory epithelium, leading to alteration of the lung function, lung inflammation, decreased mucociliary clearance, and lung histology Most e-cigarettes contain three components: 1. A solvent contains propylene glycol and vegetable glycerin which can cause specific damage to the lungs. The findings suggest that propylene glycol contains oral and nasal irritants and causes squamous cell metaplasia.”

He added: “Flavors are highly variable given the huge variety of flavors available in e-liquids. The flavoring ingredients are said to be harmful and cause brochitis obliterans. Around 2019, there was a random increase in cases of acute respiratory failure due to e-cigarette or vaping product use-associated lung injury (EVALI). E-cigarette users have been divided into two subtypes: 1. Current smokers seeking to quit traditional cigarettes: Compared to other forms of nicotine replacement therapy, e-cigarettes were found to be very effective. 2. Recreational users – Refers to recreational users, a specific group of young people who have never smoked a traditional cigarette.”

He highlighted that there are many misconceptions that:

1. E-liquid aerosols are less addictive than traditional cigarettes

2. Contains no nicotine

3. It has become a style statement among young people.

Dr. Sanket Jain concluded: “The rise in the use of electronic cigarettes has increased due to numerous factors, such as the characteristics that they are small, elegant, technologically advanced in terms of nicotine and salt formulations and also attractive flavors. There has been conflict regarding the use of electronic cigarettes as a tool for smoking cessation, but there are direct effects on the lungs due to vaping that have been proven by many studies published in various journals. Also, there is a public misconception that is playing a major role in the rise of vaping among people.”

Adding his expertise to it, Dr Sandeep Attawar, Director of Lung Transplantation at Sir HN Reliance Foundation Hospital, insisted: “Aromatizing agents have been shown to have disastrous effects on the cells and structure of the lungs, especially the lungs. sweet and cinnamon flavors.
